Absolutely an original item, not a reproduction.  This item is a "cover" in philatelic terms--a Confederate States of America item of "Postal History" to be more precise.
The original letter is not present.  You are only purchasing the postal portion.
A Confederate #11 tied by a black "ORANGE CH Va." JUL 16" circular datestamp on an envelope of obvious northern manufacture addressed to "Maj. J. Moore Wilson, Midway Hospital, Charlottesville, Va."  On the obverse are apparently contemporary pencil markings:  1)  Taking up the entire area to the left of the stamp " Major J. Moore Wilson, Midway Hospital"--with each line being underlined in double lines; 2)  to the left of the address:  "Major Yanny" which is also underlined in double lines.  On the reverse there is also light pencil below the address flap that appears to be contemporaneous:  "Major J. Wilson, 1st La Regiment"--that's my guess.
An exciting Charlottesville, Virginia hospital cover.  Mounting remnants on the top flap on the reverse.  The cover is also opened on all sides, presumably to show the watermark in the paper to illustrate that it was probably a captured blank envelope.
